With Patek Philippe Certificate of Origin dated 28 June 2000, additional case back, setting pin, product brochure, instruction manual, sales tag and leather wallet. Furthermore delivered with Patek Philippe Extract from the Archives confirming production of the present watch with black dial and Arabic numerals in 1999 and its subsequent sale on 13 March 2000. The Extract from the Archives further mentions the replacement of the leather strap to a platinum bracelet in 2011.

The present iconic watch is certainly one of the most charismatic example of a reference 5004. The present watch displays the attractive combination of mechanical complexity, dial design, and exclusivity. Most evident to the eyes of the beholder, this 5004 is fitted with an outstanding and rare platinum bracelet, granted upon request by one of Patek Philippe's most loyal client following the approval of the firm.

This exclusive and desirable bracelet, while not mentioned on the Certificate of Origin, is fully confirmed by the Extract from the Archives. The present Patek Philippe platinum bracelet was especially ordered and set in 2011 while the watch was serviced at atek Philippe in Geneva.
The present reference 5004 is furthermore in pristine condition, featuring the rare and much sought-after black dial with Arabic numerals.
